GentlePlacement is a senior care placement service working across Los Angeles County, with our roots and our deepest knowledge in the San Fernando Valley.

What makes us different

A placement can look simple on paper: match a name to a list and send a referral. Whether it works out depends on knowing what each home can really handle.

We manage 4 licensed residential care homes in the San Fernando Valley, with 22 beds between them, all cleared for non-ambulatory residents and all holding hospice waivers. We are in those buildings. We know what a 3am staffing problem looks like, what a good caregiver costs, and why a home with an open bed sometimes still should not take your parent.

If one of the homes we manage is a good fit for your parent, we will tell you that we manage it, and we will always show you other options too. The choice is always yours.

That operating knowledge is the whole product. It is why we can look at a care appraisal and tell you in a minute whether a particular home can actually handle it, and why we are usually the ones who get called about the placements that have already been declined twice.

The cases we take

  • Non-ambulatory residents who need two-person assist or a hoyer lift, which requires a home cleared for non-ambulatory residents.
  • Bariatric residents, where most homes simply do not have the equipment or the staffing ratio.
  • Residents who wander or have behavioral needs that have led to a discharge notice somewhere else.
  • Residents on hospice, who need a home with the hospice waiver so they are not moved again at the end.
  • Spanish-speaking residents, who do far better in a household where the caregivers speak their language.

Where we work

The San Fernando Valley is home. We know many of the homes here well: we have walked through them and we know which ones are run well.

Across the rest of Los Angeles County we keep state licensing records on all 1,644 licensed residential care facilities, 1,359 of which are small board and care homes, across 180 communities. Where we do not have a personal relationship with a home, we say so, and we help you get to someone who does.

How we are paid

The care home pays our placement fee after a resident moves in. Families pay us nothing, and there is no fee to the health plan or to Medi-Cal. We have had the arrangement reviewed by an attorney, including for Medi-Cal members.

We are a private placement service. We are not a contracted CalAIM Community Supports provider, we do not hold a contract with any health plan, and we are not a licensed medical provider. When a family needs Community Supports authorization we submit the referral to the contracted agencies and work alongside their care navigators.

Our data

Facility information on this site comes from the California Department of Social Services Community Care Licensing public dataset (state extract dated May 25, 2025, the newest the state had published when we last checked on October 2, 2026). We republish it because families deserve to see which homes are actually licensed, how many beds each one holds, and how long it has been operating, before anyone asks them for a deposit. Licensing status can change at any time and should be confirmed with CDSS. How we use this data.
