For hospital and SNF discharge planners, case managers, care navigators, and social workers across Los Angeles County.
Send us the situation and we come back, often the same day, with licensed homes that have a bed open now and can actually meet the care level, including residents who have already been declined somewhere else.
We check each home's fire clearance and waivers before we send you anything, so a resident who cannot evacuate on their own does not get placed in a home that legally cannot take them.
What we need from you

How fast can you turn around a referral?
Often the same day for urgent discharges. Send a short summary and we come back with confirmed options, not maybes.
Do you take Medi-Cal members?
Yes. We are not a contracted Community Supports provider ourselves, so we submit the referral to the contracted agencies and work with the assigned care navigator through authorization.
What do you do with residents who have been declined elsewhere?
Those are the ones we want. Non-ambulatory, bariatric, two-person assist, behavioral, wandering, hospice on site. We know which homes hold the right fire clearance, waivers and staffing, which is usually why the decline happened in the first place.
Is there a cost to our facility or to the member?
No. Our fee is paid by the receiving care home after move-in. There is no charge to the discharging facility, the member, the family, or the health plan.
