  • Same-day bed confirmation when the discharge clock is running
  • Private pay and Medi-Cal. We help you work out which applies.
  • The hard placements: bariatric, behavioral, two-person assist, hospice on site

How it works

Three steps, and we do most of the work

You tell us the situation

One short call. Care level, budget, area, how fast you need it, and anything other homes have already said no to. Ten minutes, usually less.

We shortlist and confirm

We check which homes actually have a bed right now, confirm they can meet the care level, and give you two or three real options with real prices, not a list of fifty names to call yourself.

You tour, we handle the rest

We set up the tours, help with the paperwork and the physician's report, and stay on it through move-in day. If it is not the right fit, we keep looking.

Why families call us

We have been inside the homes

Directories list facilities. We place people. The difference shows up on the day you need a bed for a parent who weighs 280 pounds, needs two caregivers to transfer, and is being discharged on Friday.

We also manage 4 licensed residential care homes in the San Fernando Valley with 22 beds between them, all cleared for non-ambulatory residents and all holding hospice care waivers. That is not a sales line. It is why we know what a good home actually looks like from the inside, and why we can tell when one is overstating what it can handle.

More about how we work

One call instead of fifteen

Families typically call a dozen homes and get voicemail at most of them. We already know who has space and who is at capacity.

Free to you, paid by the home

Our fee comes from the care home after move-in. Your family pays the home its standard rent and nothing extra to us. Ever.

We say no when it is wrong

If a home cannot safely handle your parent's care level, we will tell you, even when that home has a bed open and we would get paid.

Who pays

Private pay, Medi-Cal, VA, or long-term care insurance

Most families arrive assuming they will pay everything out of pocket, and a good number of them turn out to qualify for help they did not know existed.

Private pay is the most straightforward. You pay the home monthly and choose from the widest set of options.

Medi-Cal and CalAIM. Medi-Cal programs can pay for the care services in a licensed home for eligible members. They do not pay rent: the resident pays room and board from their income. We are not a contracted Community Supports provider, so we submit your referral to the contracted agencies whose care navigators handle the authorization, and we stay with it until there is an answer.

VA Aid & Attendance and long-term care insurance can both offset a meaningful share of the monthly cost. We will tell you if either looks likely in your case.

Working a discharge?

If you are a hospital or SNF discharge planner, a care navigator, or a case manager, you need a bed confirmed before the clock runs out, not a brochure. Send us the situation and we will come back with real options the same day, including for the residents who have already been declined elsewhere.

Run a licensed care home?

If you own or operate an RCFE in Los Angeles County and have beds to fill, we send pre-qualified, care-level-matched residents and you pay nothing until someone moves in.

For care home owners

Questions families ask

What is a board and care home?

A board and care home (licensed in California as a Residential Care Facility for the Elderly, or RCFE) is a regular house in a residential neighborhood licensed to care for a small number of residents, usually six or fewer. Residents get a bedroom, all meals, housekeeping, laundry, medication management and help with bathing and dressing, with caregivers on site around the clock. Because the homes are small, the staff-to-resident ratio is typically much better than in a large assisted living community, and the cost is usually lower.

How much does GentlePlacement cost my family?

Nothing. Our placement fee is paid by the care home once your loved one moves in, not by your family and not by the health plan. You pay the home its normal monthly rent, the same rent you would pay if you had found it yourself.

How fast can you find a bed?

For urgent cases we can often confirm an available licensed bed the same day. We know which homes in our network have openings right now, which is the step that normally takes families two weeks of phone calls.

What if my parent has been turned down by other homes?

That is a large part of what we do. We place residents who need two-person assist, who are non-ambulatory, who are bariatric, who wander or have behavioral needs, and who need hospice care on site. These placements take knowing which specific homes hold the right license waivers and have the right staffing. That is exactly the list we keep.

Do you work with Medi-Cal, or only private pay?

Both. Many families pay privately, and many qualify for help through Medi-Cal's Community Supports programs under CalAIM, which can pay for the care services in a licensed home for eligible members. The resident still pays room and board from their income. We are not a contracted Community Supports provider ourselves. We work alongside the contracted agencies and submit referrals to the care navigators who handle authorization, so you are not trying to navigate that system alone.

Which areas do you cover?

Our home base is the San Fernando Valley, where we know the homes personally. We maintain licensing records and contacts across all of Los Angeles County: 1,361 licensed small board and care homes in 128 cities. We can help families anywhere in the county.
