What it is

Discharges move fast. A case manager tells you on Tuesday that your parent is ready on Friday, and suddenly you need a licensed home with an open bed, the right clearance, and a rate you can afford, while also going to work and visiting the hospital.

Urgent discharges are a large part of our work. We call the homes that fit, confirm openings and clearances, and can often come back with confirmed options the same day. Moving in still needs a physician's report and TB clearance, and we help you get both quickly.

What to look for

What needs to happen, in order:

  1. Call us and the case manager today. Tell the case manager you are arranging a care home.
  2. Get the physician's report started. California form LIC 602A, completed within the past year. Ask the hospitalist to complete it before discharge.
  3. Ask for a TB clearance now. It can hold up a move if left to the last day.
  4. Get the discharge summary and medication list, including any new orders.

What it costs and who pays

Same-day placement costs your family nothing. The home's monthly rate is the same as it would be on any other day, typically $3,000 to $6,000 a month for board and care in California in 2026.

Can the hospital discharge my parent with nowhere safe to go?

Hospitals are required to plan a safe discharge. Tell the case manager you are arranging a care home and keep them updated.

What if Medicare will cover rehab first?

Medicare may cover a short skilled nursing stay after a qualifying hospital admission. If that applies, we can line up the board and care home for when rehab ends.
